A nurse-led assessment of your whole situation, including a medication review that catches dangerous combinations (polypharmacy), a home-safety evaluation, and a clear, written set of next steps. We even find the free medication programs most families never hear about.
3Point-of-Care Visits
Clinical care brought to your home at a fraction of typical dispatch pricing: blood draws and lab work, INR/blood-thinner monitoring, vitals and post-discharge follow-up, and routine point-of-care testing. For people who can't easily get to a clinic, or simply shouldn't have to.

Meet Shawnmarie Moore.
Shawnmarie is a registered nurse (RN) with hands-on experience across the hospital and the home, from the cardiac cath lab to the kitchen table. She started Rooted in Healthy Remedies because she kept seeing the same thing: families doing everything right and still falling through the cracks of a system that only looks at one patient at a time.
She's the advocate who reads the medication list nobody reconciled, makes the provider call nobody made, and finds the resource nobody mentioned, so your family gets the care it actually needs.

What is a care concierge, and how is it different from a home-health nurse?
A home-health nurse is assigned to one patient and one set of orders. A care concierge looks at the whole household — the patient, the caregiver, the medications, the home, the finances, and the logistics — and builds and coordinates one plan across all of it. We organize the care; we don't replace your doctor or your home-health team.
What areas do you serve?
Care coordination, consulting, and medication review are available remotely, nationwide. In-home visits and point-of-care services are offered locally in the Lafayette and New Iberia (Acadiana) region of Louisiana.
Do you replace my doctor or existing nurse?
No. We coordinate with your existing providers — reconciling medications, making the calls, and organizing services — so everyone is working from the same plan.
Is the first consult really free?
Yes. The first conversation is free and there's no obligation — we listen to what's happening and tell you honestly how we can help.
Do you take insurance?
Consulting and concierge services are typically private-pay, but a large part of what we do is help you use the benefits, programs, and free-medication resources you already qualify for — often saving far more than the cost of the service.
